Sponsored Residential Homes

Sponsored Residential Homes provide 24-hour residential and individuals supports for one or two adults who are intellectually and/or developmentally disabled. These individuals may be transitioning from a natural family environment or from a program that has recently closed or downsized. Sponsored Residential Homes provide each person supported with the opportunity to learn self-advocacy, choice, healthy life habits, behavioral self-management and also develop critical life skills necessary to function as independently as possible within their home and community environments. 

Sponsored Residential Consulting Package includes:

  • Introduction to the DBHDS Licensing Process Overview 

  • Initial Provider Application to include:

    • All policies and procedures including (with needed revisions)

    • Home and Community Based Services implementation 

    • Working Budget

    • Development of Organization Structure

    • Job Descriptions

    • Criminal History and Central Registry Account Set up

    • Development of sample client file (for inspection)

  • Organization setup to include:

    • Business License

    • State Corporation

    • EIN

  • Tour of a fully licensed program and the opportunity to ask the owner questions

  • Preparation for On-Site Review

  • Enrolling with DMAS (Department of Medical Assistance Services)

  • Enrollment in CHRIS (Computerized Human Rights Information System)

  • Enrollment in WAMS (Waiver Management System)

  • Overview of billing for services
